Gyda phwy y bydd yr Hybiau Cymorth Cynnar yn gweithio?
Plant, pobl ifanc a’u teuluoedd lle mae anghenion llesiant wedi’u nodi y mae angen cymorth ar y teulu i’w cyflawni. Mae'n bwysig iawn ein bod yn meddwl pa ganlyniadau y mae'r plentyn a'r teulu am eu cyflawni drwy siarad ag aelod o staff HCC.
Sut gall teuluoedd gael mynediad at gymorth yn uniongyrchol gan yr Hyb Cymorth Cynnar?
Gall plant, pobl ifanc a'u teuluoedd gael cymorth drwy ofyn am apwyntiad drwy'r ysgol i siarad â'r HCC yn gyfrinachol yn yr ysgol am yr hyn sy'n bwysig i'r teulu. Dyma fydd cychwyn y broses atgyfeirio i nodi’r gwasanaeth cywir ar yr adeg gywir i helpu teuluoedd i fyw bywydau hapus, iach a diogel.
Beth fydd Hybiau Cymorth Cynnar yn ei wneud?
Fel rhan o'r atgyfeiriad bydd HCC yn darparu gwybodaeth am ein gwasanaethau a gwasanaethau eraill a allai fod o gymorth i'r teulu. Rydym yn darparu cyngor mewn perthynas â sut y gallwn ni neu wasanaethau eraill eu cefnogi a hefyd yn darparu cymorth os yw'r gwasanaethau a ddarparwn y rhai cywir i helpu'r teulu i gyflawni eu nodau llesiant.

Who will the Early Help Hubs work with?
Children, young people and their families where there are identified wellbeing needs that the family need assistance to achieve. It is really important that we think about what outcomes the child and family want to achieve by talking to an EHH member of staff.
How can family’s access support directly from the Early Help Hub?
Children, young people and their families can access support from requesting an appointment via the school to speak to the EHH confidentially at the school about what matters to the family. This will be the start of the referral process to identify the right service at the right time to help families live happy, healthy and safe lives.
What will Early Help Hubs do?
As part of the referral EHH will provide information about our services and other services that may be of help to the family. We provide advice in relation to how we or other services can support them and also provide assistance if the services we provide are the right ones to help the family achieve their wellbeing goals.
