Cytundeb Rhieni er mwyn i'r ysgol ddosbarthu meddyginiaeth/ Parental Agreement for Education Setting to Administer Medicine

Rhaid i rieni/ gofalwyr gwblhau'r ffurflen atodedig er mwyn i'r ysgol fedru rhoi meddyginiaeth i blentyn yn ystod oriau ysgol. Dylid sicrhau mai meddyginiaeth sydd wedi ei ddosbarthu gan feddyg yn unig sy'n cael ei roi i'r ysgol.

 

Parents/ Carers must complete the above form in order for the school to administer any medication. Please note that the medication must be prescribed by a doctor in order for the school to administer the medication in school.

GWASANAETH NYRS YSGOL/ SCHOOL HEALTH NURSE SERVICE

 

Mae’r Gwasanaeth Iechyd Nyrs Ysgol yn darparu addysg, cyngor a chymorth ar draws ystod o agweddau sy’n ymwneud ag iechyd disgyblion oedran ysgol ynhyd â’r gymuned ehangach. Maent yn gweithio’n agos gyda’r ysgol i ddarparu sesiynau sy’n unol â’r cwricwlwm ABCh ac ARhPh yn y dosbarth. Mae’r nyrs hefyd ar gael i weithio un i un pe dymunir. The School Health Nurse Service provides education, advice and assistance in all issues related to health and public health for school aged children and the wider community. They will work closely with school to provide evidence based sessions in the classroom in line with P.S.E .and S.R.E. curriculum. School nurses are also available to discuss on a one to one basis if appropriate. If you wish to contact the school nurse, Nia Rogers on 07816180247/ 01792 517050.

Gall rhieni a gofalwyr a allai fod yn cael trafferth gydag ymddygiad plant gartref gael cymorth a chyngor drwy Siarad â Rhieni – Cymru. Maent wrth law i gefnogi rhieni pan fo'u hangen. Pori erthyglau ar y cwestiynau rhianta mwyaf cyffredin gan arbenigwyr. Neu siaradwch un-i-un gyda hyfforddwr rhianta cymwysedig am unrhyw beth sy'n eich poeni. Mae'r cyfan am ddim, ac nid oes unrhyw bwnc yn rhy fawr, bach nac annifyr.
